Job Summary: Conducts health assessment interviews focused on wound, ostomy, and continence-related care. Performs focused physical assessments, develops plans of care based on diagnoses and individual client needs, and recommends medications and treatments as appropriate.

Required Qualifications:

• Two (2) years of Med/Surg experience
• Wound and colostomy experience

• Associates Degree (ASN, ADN) from an accredited Nursing Program

• New York State RN License
• Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED)
• AHA BLS
• Current certification in one of the following:
  • Certified Wound Ostomy Continence Nurse (CWOCN) issued by the Wound, Ostomy and Continence Nursing Certification Board (WOCNCB)
  • Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB
  • Certified Wound Care Nurse (CWCN) issued by the WOCNCB
  • Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM)
  • Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO)
